This page is used to access the on-line problem report form for HSU's Information Technology Services Labs. Use this form to report any problem you have experienced with lab hardware, software or facilities (temperature, furniture, etc.) to the ITS staff.
Before submitting a problem report, if the problem is workstation or software related, please check that rebooting the workstation does not solve the problem.
If you do not know your HSU User Name or Password you may reactivate them online or contact the Help Desk at x4357 for assistance.
